2004-2008 5-year Consolidated Plan [pdf]. Communities receiving Community Development Block Grant and/or HOME program funds as Entitlement cities are required to develop a 5-year plan. The Overall Table of Contents and the Executive Summary of the current 2004-2008 Plan is included here for your review. Copies of the full 5-year Plan and the current Annual Plan may be picked up at the Department's office. Community Forums in development of the 2009-2013 Plan will begin in the early Fall of 2007 and be submitted to HUD in May 2008.
Annual updates of the 5-year Plan are required to be submitted 45 days before the community's Fiscal Year (May 15th of each year). The Overall Table of Contents and Executive Summary may be viewed via the links below. A full plan may be picked up at the Department's office.

Annual Performance Report
Annual Performance Report for 2006 [pdf]. A Consolidated Performance and Evaluation Report (CAPER) is prepared and submitted to the U.S. Department of Housing and Urban Development each Fall, no later than September 28th. The most recent report is provided.

Continuum of Care
The purpose of HUD Continuum of Care Homeless Assistance Programs is to reduce the incidence of homelessness in communities by assisting homeless individuals and families to move to self-sufficiency and permanent housing. The City of Winston-Salem, in cooperation with Winston-Salem/Forsyth County Council on Services for the Homeless and its member agencies, coordinates annual Continuum of Care submissions.
