How to File a Claim (for Employees Enrolled in Short Term Disability Insurance)

  • Step 1: Know when it's time to file. If you're absent from work, The Hartford can advise you on when to file your claim. If the absence is scheduled, such as an upcoming hospital stay, call prior to your last day at work. If unscheduled, please call The Hartford at your earliest convenience.
  • Step 2: Have the following information ready:
    • Name, Address, Phone Number, and other key identification information
    • Name of your department and the last day of active full-time work
    • The nature of your claim
    • Your treating physician's name, address, and phone and fax numbers
  • Step 3: Make the call or visit The Hartford online. With your information handy, call The Hartford at 1-866-945-4558. The Hartford is open between 8 a.m. and 8 p.m. Eastern Time from Monday through Friday. You'll be assisted by a nurse with an average of 20 years of clinical experience. Your nurse has the expertise to answer questions, tell you what to expect next, and guide you through the disability process. Your nurse can also talk to your doctor directly if necessary. So feel free to share your concerns.

If you prefer to file your claim online, go directly to The Hartford's website and make sure to have the information in Step 2 above readily available. After you register with the website on your first visit, you may file all your claims online if desired.
